2. Activate your PRIMA plan to earn one (1) e-raffle entry. One activated plan is equivalent to one raffle entry. The more plans you buy and activate, the more chances of winning.*
3. Only activated and registered PRIMA plans during the promo period from August 1 to October 31, 2026, will be considered for the raffle. An individual can only win once, and in case their name is drawn twice, they’ll get whichever prize is higher.
4. Upon successful activation of the PRIMA product, register your personal details and card information for verification purposes on this Google Form.
5. The electronic raffle will be drawn on November 6, 2026.
6. There will be one winner each for the ₱100,000, ₱200,000, and ₱300,000 prize categories. Maxicare Healthcare Corporation will shoulder the 20% withholding tax on the cash prizes.

Raffle winners will be notified via their registered mail, email, SMS, and phone call. They have sixty (60) days from the receipt of the SMS, mail, and email notification to claim their prize.
