How to Activate your PRIMA Access by MaxiHealth
After your purchase, you need to activate your PRIMA Access eVoucher with the Reference Code before you can avail the benefits.
*The membership registration period is up to three (3) months from the date of purchase.
- Go to Dashlabs’ registration link
- Enter the Reference Code you’ll receive by email and complete your details
- Once successfully registered, you will receive an email together with your PRIMA Access by MaxiHealth electronic card (eCard) within 24 hours.
You may start enjoying your benefits once you receive your eCard!

Note:
- A referral to a specialist is required and will only be issued after an initial consultation with a General Practitioner (GP) or Internal Medicine (IM) doctor. However, members may also see a specialist if they present a valid outside doctor referral.
- Laboratory tests can be scheduled but must be endorsed by a PCC doctor. Tests outside the plan coverage will be out-of-pocket. Members may also undergo laboratory tests if they present a valid outside doctor’s request

- Present your PRIMA Access by MaxiHealth card or eCard, along with 1 valid ID with a photo upon availing of the services offered in this healthcare plan
- No card or eCard, no availment
- Membership is valid up to 1 year from the date of activation
- The card is non-transferable once registered

- Free cancellation and refund within 15 days if the product has not been used
