BASIC ACCREDITATION REQUIREMENTS
Other important requirements are as follows:
- Filled- up Maxicare Accreditation Application form (online application)
- Photocopy of TIN and SSS ID
- Any of the following Clearance : NBI/ Police/ Barangay
- (2) copies Passport sized Picture and 1×1 sized picture IN BLUE BACKGROUND
- Photocopy of Two (2) Valid Identification Cards
- Insurance Commission License (if applicable)
- Interview with the Accreditation Officer or Sales Manager
